Mitotan (Lysodren) instructions

Indications:

This product is suitable for the treatment of the following patients:

Inoperable, functional and non-functional adrenocortical carcinoma, adrenal cortical hyperplasia and hypercortisolism caused by tumors.


Usage and Dosage:

For children under 18 years old, the efficacy and safety of this drug have not yet been fully determined. Doctors will determine the daily dose based on the patient's weight or body surface area.

The daily dose for adults is 1 to 6 grams (taken in 3 to 4 times), and can be gradually increased to 8 to 10 grams per day. The generally recommended maximum daily dose is 18 grams.


Medicine contraindications:

Those who have allergic reactions to this drug should not take it.


Drugs Note:

For patients with mild to moderate liver or kidney impairment, the dose may need to be adjusted downwards based on blood concentrations of the drug.

This drug is not recommended for patients with severe liver or kidney damage.


Side effects and adverse reactions:

The most common side effects of Mitotane include: depression of the central nervous system, dizziness, drowsiness, skin rash, loss of appetite, nausea, vomiting, diarrhea, etc.

Other side effects: headache, high blood pressure, orthostatic hypotension, depression, confusion, muscle tremors, hemorrhagic cystitis, etc.


Drug interactions:

1. Spironolactone (spironolactone) can reduce the efficacy of this drug, so its simultaneous use is not recommended.

2. Alcohol, first-generation antihistamine drugs (sedative antihistamine), sedatives and hypnotics, morphine analgesics, anti-epileptic drugs, etc. have a depressant effect on the central nervous system. Their combined use can increase related side effects such as drowsiness and dizziness.

3. This drug can increase the liver's metabolism of certain drugs (such as some anti-epileptic drugs, barbiturates, Warfarin, etc.), and combined use can reduce the efficacy of the latter, so special monitoring is required.

4. Medication users should consult their attending physician before using other medications.


Notes:

1. This drug is a chemotherapy drug. Care must be taken when handling and discarding the drug.

2. Taking this medicine with a full stomach can increase absorption.

3. This drug can suppress the central nervous system and cause drowsiness, dizziness, etc. Therefore, while taking the drug, try to avoid driving or operating machinery and other activities that require high concentration.

4. If the person taking the medicine develops infection, trauma or other diseases, he should seek medical treatment immediately to avoid acute adrenocortical insufficiency.

Therapeutic efficacy:

The efficacy of mitotane in advanced ACC was evaluated in a clinical trial. The researchers included a total of 127 patients with advanced ACC who were treated with mitotane monotherapy. Progression-free survival and overall survival (PFS, OS) were assessed with Kaplan-Meier.

The study results showed that 26 patients (20.5%) experienced objective responses, of which 3 patients achieved complete remission. Overall, the median PFS was 4.1 months (range 1.0-73) and the median OS was 18.5 months (range 1.3-220).
