English name: Carbamazepine
Function and indications
Partial seizures of epilepsy: complex partial seizures, simple partial seizures.
Primary or secondary generalized tonic-spasmodic attacks.
Mixed seizures.
It can be taken alone or in combination with other anticonvulsant drugs. It is ineffective against absence seizures and myotic seizures.
Trigeminal neuralgia
Trigeminal neuralgia caused by multiple sclerosis
Primary trigeminal neuralgia
Primary glossopharyngeal neuralgia
Usage and dosage
This product can be taken with a small amount of liquid during meals, after meals or between meals. In elderly patients, the dose of carbamazepine should be chosen carefully.
Epilepsy
Carbamazepine should be used as a monotherapy whenever possible. Treatment should start with a small dose and slowly increase to obtain the best effect. When the attack is controlled, it can be slowly reduced to the lowest effective dose. Measuring blood drug concentrations can help determine appropriate dosage. If carbamazepine is added to other anti-epileptic drugs, the dose of carbamazepine should be gradually increased while maintaining the original drug dose, and the doses of other anti-epileptic drugs can be adjusted if necessary (see [Drug Interactions]).
Adults
The initial dose is 100-200 mg each time, 1-2 times a day, and gradually increase the dose until the best effect (usually 400 mg, 2-3 times a day). Some patients rarely need to increase to 1600 mg per day.
Children
10-20mg/kg body weight per day. 100-200mg/day for children under 12 months old, 200-400mg/day for 1-5 years old. 6-10 years old, 400-600mg/day. 11-15 years old, 600-1000mg/day, taken in divided doses. For children 4 years old or younger, the recommended initial dose is 20-60 mg/day, and then increase by 20-60 mg every other day. For children over 4 years old, the initial dose can be 100 mg/day, then increase by 100 mg weekly.
Trigeminal neuralgia
The initial dose is 200-400mg/day, gradually increase until pain relief (usually 200mg each time, 3-4 times a day), and then gradually reduce the dose to the lowest maintainable dose. The recommended initial dose for elderly patients is 100mg each time, 2 times a day. See the instructions inside the package or follow your doctor's advice for details.
Adverse reactions
1. Especially in the early stages of treatment with carbamazepine, or when the initial dose is too large or in elderly patients, some adverse reactions may occasionally or often occur, such as: central nervous system adverse reactions (dizziness, headache, ataxia, drowsiness, fatigue, diplopia); gastrointestinal discomfort (such as nausea, vomiting) and skin allergic reactions.
2. Dose-related adverse reactions usually resolve on their own within a few days or after reducing the dose. Adverse reactions in the central nervous system may be a manifestation of excessive dosage or significant fluctuations in blood concentration. In this case, blood levels should be monitored, the daily dose should be reduced and divided into 3-4 doses.
3. Adverse reactions are listed in the table below (Table 1) according to their frequency of occurrence, using the following expressions: very common (≥1/10); common (≥1/100, 1/10) ; Uncommon (≥1/1,000;1/100); Rare (≥1/10,000;1/1,000); Very rare (1/10,000), including case reports.
*There are also rare reports in some Asian countries, please refer to [Notes] Adverse drug reactions from spontaneous reports and literature (frequency of occurrence unknown)
4. The following adverse drug reactions have been reported in spontaneous case reports and literature after the launch of Delido. Because the size of the patient population from which these spontaneous reports originate is unclear, incidence cannot be reliably estimated and is therefore classified as unknown frequency. Adverse drug reactions are listed according to MedDRA system organ classification. Within each system organ class, order in descending order of severity.
5. Abnormal immune system. Drug eruption accompanied by eosinophilia and systemic symptoms (DRESS).
6. Skin and subcutaneous tissue abnormalities Acute generalized exanthematous pustulosis (AGEP).
Precautions
Hematological effects, severe skin reactions, pharmacogenomics, limitations of genetic screening, allergic reactions, other skin reactions, seizures, liver function, kidney function, etc. (See the inner packaging instructions for details) Please read the instructions carefully and use according to your doctor's advice.
Contraindications
·Those who are known to be allergic to carbamazepine and related structural drugs (such as tricyclic antidepressants) or other ingredients of the preparation. ·Patients with atrioventricular conduction block. · Serum iron is severely abnormal.
·Patients with a history of bone marrow suppression.
·Patients with a history of hepatic porphyria (such as acute intermittent porphyria, variant porphyria, porphyria cutaneum tarda), severe liver insufficiency, etc.
·Theoretically, carbamazepine (which is structurally similar to tricyclic antidepressants) should be avoided in combination with monoamine oxidase inhibitors (MAOIs) (see [Drug Interactions]). Discontinue monoamine oxidase inhibitors for at least two weeks before taking carbamazepine, or longer if clinical circumstances warrant.
