Sopyrronium Bromide (Sofdra) for Hyperhidrosis: Uses, Mechanism and Facial Application Risks

　　Sopyrronium bromide is a topical anticholinergic medication.Its ointment formulation(brand name Sofdra)is currently approved for the treatment of primary axillary hyperhidrosis,commonly known as excessive underarm sweating.Its core mechanism of action is to selectively inhibit M3 cholinergic receptors on sweat gland cell membranes,blocking the stimulatory signal of acetylcholine to sweat glands,thereby effectively reducing sweat secretion.It is important to note that this medication is currently only approved by regulatory authorities for use on the underarm area and has not been approved for the treatment of hyperhidrosis on the face or other parts of the body.The original overseas version is priced at approximately$1,000 per tube.

　　Strictly from a medical and clinical practice perspective,it is strongly not recommended to apply sopyrronium bromide ointment to facial skin,based on three core considerations.First,there are significant differences in the density and types of sweat glands between the face and underarms.The face is dominated by eccrine glands,while the underarms have both apocrine and eccrine glands.The efficacy and safety of the drug for facial sweat glands have not been systematically validated through clinical research,and therapeutic effects cannot be guaranteed.Second,facial skin is thinner and more delicate than underarm skin,resulting in higher transdermal absorption of the drug.Excessive absorption may cause systemic anticholinergic side effects,including dry mouth,blurred vision,urinary retention,and tachycardia.Third,the ointment base is formulated specifically for the characteristics of underarm skin.Application to the face may clog hair follicles,leading to skin problems such as acne or contact dermatitis.To date,no international clinical guidelines or drug labels support its use for the treatment of facial hyperhidrosis.

　　If a patient attempts to use this medication on the face without medical advice,the following safety precautions must be strictly followed:Perform a small patch test on the postauricular skin before use and observe continuously for 24 hours to confirm no allergic reactions such as redness or stinging before cautious use.If severe skin dryness or burning sensation occurs after application,immediately rinse the affected area thoroughly with clean water and discontinue use.Strictly avoid contact with the eyes,mouth,and nasal mucous membranes during application.Due to the lack of relevant safety data,pregnant women,children,and patients with glaucoma are absolutely prohibited from using this medication on the face.

　　For patients with facial hyperhidrosis,there are currently several clinically proven safe and effective treatment options available,including botulinum toxin injections,iontophoresis,or prescription oral anticholinergic medications after evaluation by a physician.Considering all the above factors,from the perspectives of medication safety and therapeutic efficacy,it is strongly not recommended to use sopyrronium bromide ointment for the treatment of facial hyperhidrosis.
