  Newly diagnosed or previously treated Ph+CML in chronic phase(adult patients):The recommended dose is 80 mg orally once daily,or 40 mg orally twice daily.The choice between regimens is determined by the treating physician based on tolerability and concomitant medications.

  Ph+CML in chronic phase with T315I mutation(adult patients):The recommended dose is 200 mg orally twice daily.

  Regardless of the regimen,asciminib tablets should be swallowed whole—do not crush or chew.To optimize absorption and minimize gastrointestinal variability in drug exposure,administration should occur on an empty stomach:avoid food for at least 2 hours before and at least 1 hour after dosing.

  3.Missed-Dose Management

  Missed doses are common in long-term targeted therapy,and proper handling is essential.Asciminib's make-up dosing strategy differs by daily frequency:

  Once-daily regimen(80 mg):If a dose is missed and more than 12 hours remain until the next scheduled dose,take the missed dose immediately.If fewer than 12 hours remain,skip the missed dose and take the next dose at the regular time.Never take a double dose to make up for a missed one.

  Twice-daily regimen(40 mg or 200 mg):If a dose is missed and it is within 4 hours of the next scheduled dose,take the missed dose immediately.If more than 4 hours have passed,skip the missed dose and take the next dose on schedule.Doubling the dose is strictly prohibited.

  To minimize missed doses,patients are encouraged to use a pill organizer and set timed reminders on their phone.Frequent missed doses should be discussed with the treating physician to evaluate adherence strategies.

  4.Safety Monitoring and Precautions

  While asciminib delivers precise targeted benefits,it carries adverse effects that require proactive management:

  Hematologic toxicity:Thrombocytopenia and neutropenia are the most common dose-limiting toxicities.Regular complete blood counts are necessary;grade 3 or 4 cytopenias may require dose interruption or reduction.

  Pancreatic toxicity:Asciminib may elevate lipase and/or amylase levels and can trigger pancreatitis.Patients experiencing abdominal pain,persistent nausea,vomiting,or back pain should seek immediate medical attention.Baseline and periodic serum lipase and amylase testing is required.

  Hepatic monitoring:Periodic assessment of ALT,AST,and bilirubin is needed to detect drug-induced liver injury.

  Drug interactions:Asciminib is metabolized by CYP3A4.Concomitant use with strong CYP3A4 inhibitors(e.g.,ketoconazole,clarithromycin)may increase exposure;use with strong CYP3A4 inducers(e.g.,rifampin,carbamazepine)may reduce efficacy and should be avoided.

  Reproductive safety:Women of childbearing potential must use effective contraception during treatment and for at least 14 days after the final dose.Sexually active males with female partners of childbearing potential should also use barrier contraception.

  Patients on long-term therapy should establish a regular follow-up schedule,with comprehensive evaluations every 3 to 6 months,including complete blood count,comprehensive metabolic panel,bone marrow morphology,and BCR-ABL1 transcript level monitoring,to ensure sustained efficacy and safety.
