Febuxostat was started at 10 mg/day for 2 weeks, then increased to 20 mg/day for 4 weeks, then to 40 mg/day. Starting with a low dose and gradually increasing the dose can prevent acute gout attacks caused by initial changes in blood uric acid levels. Treat with febuxostat 40 mg/day for 6 months, and check the uric acid level regularly. Generally, it takes 6 months to clean up excess blood uric acid in the body. 

After 6 months of treatment:

(1) If the blood uric acid level remains between 240 umol/l and 360 umol/l, maintain febuxostat 40 mg/day treatment.

(2) If the blood uric acid level is higher than 360 umol/l, increase the dose of febuxostat by 60 mg/day, up to a maximum of 80 mg/day, and monitor blood uric acid, liver and kidney function regularly.

(3) If the blood uric acid level is lower than 240 umol/l, the dose of febuxostat can be reduced to 20 mg/day and the treatment can be maintained for 6 months.

Before taking it, check the uric acid value and keep a record. Start taking it from 10mg or 20mg. After taking it for two weeks, go to the hospital to check the uric acid value and keep a record. If the uric acid value is greater than 400, you need to increase the dosage and double the amount before. In other words, you took 10mg before and now you have to take 20mg. After taking it for about 6 weeks, test the uric acid value again to see if the uric acid value has dropped. If it has dropped, it means that this dose is effective for you. You can choose to increase the dose, or you can choose to maintain this dose and continue taking it. The purpose is to take the medicine until the uric acid value is stable between 330-360. At this time, the dosage can be reduced. During the reduction period, the uric acid value needs to be checked every few weeks. After stabilizing, it can be taken every other day. After stabilizing for a period of time, the medicine can be stopped. Therefore, while taking febuxostat, the uric acid value is a very important marker and should be monitored frequently. The maximum daily dose cannot exceed 120mg.
