1. Hydrochlorothiazide: No dose adjustment required

Thiazide diuretics (hydrochlorothiazide, indopamide) can increase serum uric acid concentrations. Patients with hypertension and hyperuricemia should avoid taking thiazide diuretics as much as possible. 

2. Azathioprine or mercaptopurine: combined use is prohibited

Febuxostat is a xanthine oxidase inhibitor that reduces serum uric acid concentration by inhibiting uric acid synthesis. Febuxostat may increase plasma concentrations of azathioprine or mercaptopurine, resulting in toxicity. 

3. Theophylline: Monitor the blood concentration of theophylline

Febuxostat may increase the plasma concentrations of theophylline. The inhibitory effect of febuxostat on xanthine oxidase can lead to an approximately 400-fold increase in the urinary excretion of 1-methylxanthine (a major metabolite of theophylline). 

4. Statins: Pay attention to muscle toxicity

According to literature reports, 3 patients who had been taking statin anti-lipidemic drugs for many years (one case each of rosuvastatin, atorvastatin and simvastatin) developed symptoms of rhabdomyolysis after adding febuxostat. After recovery, they did not relapse after taking statins again.
