【Drug name】Febuxostat
[Pharmacological Actions] Febuzotostat is a xanthine oxidase inhibitor that achieves efficacy by reducing serum uric acid.
[Indications] It is suitable for the long-term treatment of hyperuricemia in patients with gout. Not recommended for use in asymptomatic hyperuricemia.
[Usage and Dosage] Febuxostat starts at 10 mg/day for 2 weeks, then increases to 20 mg/day for 4 weeks, and then increases to 40 mg/day. Starting with a low dose and gradually increasing the dose can prevent acute gout attacks caused by initial changes in blood uric acid levels. Treat with febuxostat 40 mg/day for 6 months, and check the uric acid level regularly. Generally, it takes 6 months to clean up excess blood uric acid in the body.
[Side Effects] Common headaches, diarrhea, back pain, joint pain, rash, elevated transaminases, etc.
【Notes】
1. An increase in gout attacks may be observed after initiation of treatment with febuxostat; this is caused by the activity of deposited urate due to a decrease in changing serum uric acid levels. To prevent gout attacks when febuxostat is administered, concomitant administration of an NSAID or colchicine is recommended.
2. In randomized controlled studies, patients taking febuxostat were more likely to have cardiovascular thrombotic events (cardiovascular death, non-fatal myocardial infarction, non-fatal stroke) than patients taking allopurinol. The reasons are not yet clear. Monitor for signs and symptoms of myocardial infarction (MI) and stroke.
3. In randomized controlled studies, it was observed that transaminase levels were higher than 3 times the upper limit of normal (increased in patients treated with febuxostat and allopurinol, respectively, AST: 2%, 2%, ALT: 3%, 2%). No dose-response relationship was found for the increase in transaminases. Liver function laboratory analysis recommends treatment with febuxostat for 2 and 4 months, followed by periodic treatment thereafter. [Drug Interactions] Febuxostat is a xanthine oxidase (XO) inhibitor. Although the interaction of febuxostat with drugs metabolized by XO (such as chophylline, mercaptopurine, azathioprine) has not been studied, the inhibitory effect of febuxostat on XO will increase the plasma concentration of these drugs and cause toxicity. It is contraindicated for use by patients taking azathioprine, mercaptopurine or chophylline.
