Common side effects of bedaquiline and how to deal with them
Bedaquiline is a new anti-tuberculosis drug that has been used in recent years to treat multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). Its mechanism is different from traditional first-line or second-line drugs. It inhibits the activity of mycobacterial ATP synthase, thereby blocking the energy supply of bacteria, leading to the death of the bacteria. Bedaquiline is considered an important breakthrough in the treatment of refractory tuberculosis, especially for patients who are resistant to traditional drugs such as isoniazid and rifampin. Since the drug has a long half-life and can maintain long-term effects in the body after being taken once, it is usually taken 5 days/week for about 24 weeks, and is used in combination with other anti-tuberculosis drugs to reduce the occurrence of drug-resistant strains and improve efficacy.

In practical applications, bedaquiline is relatively well tolerated, but there may still be some side effects that warrant caution. The more common adverse reactions include nausea, joint pain, headache, abnormal liver function, and QT interval prolongation. Prolonged QT interval is a serious electrophysiological change in the heart, which may cause arrhythmia if not monitored or treated in time. Therefore, patients need regular electrocardiographic monitoring during the use of bedaquiline, especially if other drugs that may affect cardiac conduction function are used concomitantly, such as fluoroquinolones or clofazimine. In addition, electrolyte imbalance (such as low potassium and low magnesium) will also increase the risk of QT prolongation, so it is necessary to closely monitor blood potassium and blood magnesium levels and correct them if necessary.
At the same time, patients need to avoid drinking alcohol to reduce the burden on the liver, and avoid combining it with other potentially hepatotoxic drugs. As for minor adverse reactions, such as fatigue and loss of appetite, in most cases it can be alleviated by adjusting lifestyle and improving nutritional status. If necessary, symptomatic treatment can be given under the guidance of a doctor.
