Standard usage and dosage of obeticholic acid
The usage and dosage of Obeticholic acid (Obeticholic acid) need to be individually adjusted based on the patient’s severity of illness and tolerance to the drug. The drug is administered as an oral tablet, usually once daily, and is recommended to be taken at a fixed time to maintain stable drug concentrations in the body. For the main indication of primary biliary cholangitis (PBC), if the patient has an insufficient response to or is intolerant to first-line treatment ursodeoxycholic acid (UDCA), obeticholic acid may be considered as a monotherapy or in combination with UDCA under the guidance of a doctor.

The initial recommended dose is usually 5 mg per day. After observing the efficacy and tolerability, it can be gradually increased to 10 mg per day based on the doctor's evaluation. Dosage adjustments must be made with caution, especially when discomfort such as itching and elevated liver enzymes may occur in the early stages of treatment. Doctors may recommend maintaining a low dose or taking it every other day to avoid further damage to liver function. During use, liver function indicators, including ALT, AST, ALP and bilirubin levels, need to be closely monitored to determine the efficacy and adverse reactions of the drug.
For patients with moderate to severe hepatic impairment (Child-Pugh B or C), a more conservative starting dose is recommended, generally 5 mg per week, with careful evaluation of the need for higher doses. If the dose is too high, it may lead to excessive accumulation of bile acids, aggravate cholestasis or cause liver damage. In addition, obeticholic acid cannot be used in combination with certain patients with severe biliary obstruction or end-stage liver disease, and the condition must be carefully evaluated before use.
During the medication period, patients should avoid arbitrarily increasing or decreasing the dose or interrupting treatment. If severe itching or other obvious adverse reactions occur, they should report to the doctor in time so that the treatment strategy can be adjusted or symptomatic and supportive drugs can be added.
