How many vials of zotuximab are needed for each injection?
Zolbetuximab (zolbetuximab-clzb)-VYLOY is an injectable drug that can be combined with chemotherapy regimens containing fluoropyrimidine and platinum. It is suitable for patients with locally advanced unresectable or metastatic human epidermal growth factor receptor. First-line treatment of adults with HER2-negative gastric or gastroesophageal junction (GEJ) adenocarcinoma whose tumors are claudin (CLDN) 18.2-positive as determined by an FDA-approved test. Each vial of zotuximab is a lyophilized powder containing 100 mg of active drug. When administering, the total dose needs to be calculated based on the patient's body surface area (BSA) to determine the number of bottles used each time.
For example, for a patient with a body surface area of 1.8 square meters, the initial intravenous dose is 800mg/m², that is, the total dose is 1.8* 800 = 1440mg requires about 15 vials of medicine (100mg each); subsequent doses are 600mg/m² (i.e. 1080mg, approximately 11 vials) or 400mg/m² (i.e. 720mg, approximately 8 vials), depending on whether the dosing cycle is once every 3 weeks or every 2 weeks.

It is worth noting that zotuximab must be stored refrigerated between 2°C and 8°C to avoid freezing. During transportation and storage, it should be placed in the original packaging to avoid light affecting drug stability. Before use, a special diluent should be used to prepare a solution suitable for intravenous infusion. The preparation process must be carried out in a sterile environment and completed by experienced medical staff.
In actual medication, the number of vials required for each patient depends on their specific body surface area (BSA) rather than a uniform dose. This requires doctors to recalculate before each treatment to ensure that the drug dose is sufficient to achieve efficacy while minimizing toxic effects. When zotuximab is used in combination with chemotherapy drugs, the dosage regimen needs to be closely coordinated with the treatment cycle to maintain treatment continuity and tumor control.
Due to its high price and large dosage, the calculation of the number of vials of zotuximab is also an important reference factor in patient economic burden assessment and medical insurance negotiation. Therefore, clarifying the required number of vials is of great significance for patient treatment planning, drug procurement and guidance on rational drug use.
