Which drugs work best with bedaquiline?
Bedaquiline is a drug used to treat drug-resistant tuberculosis (TB), especially in patients with multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). It inhibits the growth of bacteria by inhibiting ATP synthase and preventing the energy synthesis of tuberculosis cells. In combination therapy, bedaquiline is often used with other anti-tuberculosis drugs to enhance efficacy, reduce the risk of drug resistance, and improve cure rates.
In the treatment of multidrug-resistant tuberculosis (MDR-TB), bedaquiline is often used in combination with **rifampicins (such as rifamycins)**. This class of drugs inhibits the reproduction of tuberculosis branching molecules by interfering with their RNA synthesis. Rifampin drugs have broad-spectrum anti-tuberculosis effects and can help control and eliminate bacteria. Especially in cases of drug-resistant tuberculosis, the combined use of bedaquiline and rifampicin drugs can enhance the anti-tuberculosis effect. For highly drug-resistant tuberculosis strains, combination therapy can provide a more comprehensive antibacterial effect and reduce the risk of resistance to a single drug.

Fluoroquinolones (such as levofloxacin or moxifloxacin) are important drugs in the treatment of drug-resistant tuberculosis. The combination of bedaquiline and fluoroquinolones is very common in the treatment of drug-resistant tuberculosis. Fluoroquinolones inhibit bacterial growth by inhibiting the replication and repair of bacterial DNA. The combination of bedaquiline and fluoroquinolones provides a potent anti-tuberculosis effect, especially in cases of resistance to other drugs, and this combination therapy can significantly improve the success rate of treatment.
When treating extensively drug-resistant tuberculosis (XDR-TB), bedaquiline often needs to be used in combination with other second-line anti-tuberculosis drugs (such as cycloserine, amikacin, kanamycin, etc.). These drugs are often not included in standard anti-TB treatment regimens and work against the more drug-resistant TB bacteria. Bedaquiline, used in combination with these drugs, can work synergistically through multiple mechanisms to enhance the anti-tuberculosis effect of the drugs and reduce bacterial resistance.
