Imatinib/Gleevec Correct Usage and Dosage Guidelines
Imatinib/Gleevec (Imatinib) is a tyrosine kinase inhibitor (TKI), mainly used to treat Philadelphia chromosome-positive chronic myelogenous leukemia (CML) and gastrointestinal stromal tumor (GIST). Correct use of imatinib is essential to improve efficacy and reduce side effects, so patients must fully understand the dosage guidelines and medication methods before taking it.
The recommended dose of imatinib varies depending on the type of disease. For chronic myelogenous leukemia (CML), the standard starting dose for adult patients is 400 mg once daily with food to reduce gastrointestinal discomfort. For gastrointestinal stromal tumors (GIST), the recommended dose is also 400 mg once daily. For drug-resistant or progressive disease, the dose can be increased to 600mg or 800mg (taken in two divided doses), but adjustments need to be made after physician evaluation. When taking the medicine, swallow the tablet whole, avoid chewing or crushing, and take it with a large glass of water to ensure stable absorption of the medicine. In addition, grapefruit and its juice should be avoided while taking the drug because it may affect the metabolism of imatinib and thus alter the efficacy of the drug.

During long-term use of imatinib, patients may experience different side effects, such as edema, muscle cramps, gastrointestinal discomfort, and rash. Some patients may experience bone marrow suppression, such as leukopenia or thrombocytopenia, and therefore require regular blood routine monitoring. For patients with abnormal liver function, doctors may adjust the dose or suspend medication depending on the situation. Additionally, patients with impaired renal function or elderly patients may require additional monitoring to reduce the risk of drug accumulation.
If a patient misses a dose of imatinib, it should be taken as soon as possible that day, but if it is close to the next dose, the missed dose should be skipped to avoid doubling. If you have serious adverse reactions after taking the medicine, such as severe allergic reactions on the skin, difficulty breathing, or abnormal heart rhythm, you should seek medical attention immediately.
