Can’t I stop taking obeticholic acid? Is there any problem of long-term dependence or rebound after discontinuation?
Obeticholic acid (Obeticholic) is a drug used to treat primary biliary cholangitis (PBC). It mainly slows down the synthesis of bile acids and improves liver function by activating the farnesoidX receptor (FXR). Due to the particularity of its mechanism of action and therapeutic effect, many patients may be concerned about whether dependence or rebound will occur after the drug is discontinued when using obeticholic acid for a long time.
When obeticholic acid is used to treat primary biliary cholangitis, long-term use is usually required to achieve good therapeutic effects. PBC is a chronic liver disease that progresses slowly and is difficult to completely cure. Obeticholic acid controls disease progression by regulating bile acid metabolism and reducing liver damage. Long-term administration is usually required to maintain the effect. In most cases, doctors recommend that patients continue taking obeticholic acid, especially early in treatment, until symptoms improve significantly or liver function is stabilized.

Obeeticholic acid itself is not "dependent" in the traditional sense, but patients may feel the drug's disease-controlling effect after long-term use of the drug. If the drug is stopped suddenly, it may lead to worsening of disease symptoms or incomplete recovery of liver function. This exacerbation is not due to drug dependence, but rather to the uncontrolled pathological progression of primary biliary cholangitis after drug use is discontinued. To avoid this situation, patients should gradually adjust or stop medication under the guidance of their doctor rather than making their own decisions.
The problem of rebound after stopping the drug is relatively rare in the use of obeticholic acid, but there is a certain risk. If the patient is highly dependent on drugs during treatment, or the liver function has not fully recovered, stopping the drug may cause a rebound of the disease, manifested by symptoms such as increased liver enzymes, increased fatigue, and jaundice. Therefore, doctors need to strictly monitor changes in liver function and symptoms when discontinuing the drug, and may need to gradually reduce the dose to avoid sudden worsening of the condition.
