What are the precautions for taking landiolol?
Landiolol is an ultra-short-acting adrenergicβ1 receptor antagonist. It is mainly used to treat patients with supraventricular tachycardia (including atrial fibrillation and atrial flutter). It can quickly reduce the ventricular rate. However, during use, special attention must be paid to possible adverse reactions to ensure patient safety.
1. Risk of hypotension, slow heart rate and heart failure: Landiolol may cause hypotension, slow heart rate and heart failure. Because its mechanism of action involves blocking sympathetic nerve excitation, it may induce a decrease in blood pressure and a decrease in heart rate, especially in patients with inherently hemodynamic instability or compromised cardiac function. Therefore, blood pressure and heart rate should be closely monitored during use. If obvious discomfort or condition worsens, the dose should be adjusted in time or even discontinued to prevent more serious circulatory system suppression.
2.Risk of worsening respiratory problems: This drug may worsen respiratory diseases, especially in patients with reactive airway diseases (such as asthma, chronic obstructive pulmonary diseaseCOPD). Although landiolol is relatively β1-selective, it may still affect bronchial smooth muscle and induce bronchospasm. Therefore, when used in such patients, it should be titrated to the lowest effective dose possible and changes in the condition should be closely observed. If the patient develops obvious dyspnea, the drug should be stopped immediately, and if necessary, a β2-receptor agonist (such as albuterol) should be used to relieve symptoms, and heart rate changes should be monitored at the same time.
3. Diabetes and Hypoglycemia: Landiolol may affect blood sugar levels and mask symptoms of hypoglycemia, especially in people with diabetes. Beta blockers may weaken the sympathetic nervous responses common to hypoglycemia, such as palpitations, tremors, and sweating, making it more difficult for patients to detect hypoglycemia. Therefore, patients with diabetes should closely monitor blood sugar when using landiolol and adjust the dose of antidiabetic drugs as needed to prevent the occurrence of hypoglycemic crisis.
4. Avoid sudden discontinuation:Sudden discontinuation of β-blockers may lead to excessive sympathetic nerve excitation, aggravation of angina in patients with coronary heart disease, and even myocardial infarction and ventricular arrhythmia. Therefore, when stopping the drug, the dose should be gradually reduced to reduce the impact on the cardiovascular system, and the patient should be closely observed for signs of myocardial ischemia, such as chest pain, shortness of breath, or arrhythmia, to ensure the safety of the drug withdrawal process.
