The three most taboo fruits for bosutinib/bosutinib
Bosutinib/Bosutinib is a treatment drug for Philadelphia chromosome-positive chronic myelogenous leukemia (Ph+ CML). During its use, special attention needs to be paid to the interaction with certain foods, especially fruits. The most contraindicated fruits for patients being treated with bosutinib include grapefruit, grapefruit juice, and any supplement containing grapefruit extract. These foods may significantly affect drug metabolism and efficacy.
Grapefruit is considered the most risky fruit during bosutinib treatment. Grapefruit contains a compound that inhibits the activity of the CYP3A4 enzyme, which is responsible for the metabolism of many drugs in the liver and intestines, including bosutinib. When patients eat grapefruit, it significantly decreases the rate of metabolism of bosutinib, causing its concentration in the body to increase. This increased drug concentration may cause a series of side effects, such as nausea, vomiting, abdominal pain, etc., and may even cause liver damage and bone marrow function suppression in severe cases, thus increasing the risk of anemia, bleeding and infection. Therefore, patients must strictly avoid eating grapefruit while taking bosutinib to ensure the safety and effectiveness of the drug.

Many patients may choose fruit juice as a substitute in their diet. However, grapefruit juice will not reduce the ingredients contained in grapefruit that inhibit CYP3A4. Instead, its concentrated form may further increase the impact on drug metabolism. Studies have shown that even small amounts of grapefruit juice may interfere with the normal metabolism of bosutinib, leading to increased drug concentrations. Therefore, patients are advised to completely avoid grapefruit juice while taking bosutinib to prevent potential drug interactions.
Any supplements containing grapefruit extract should also be contraindicated. These supplements are often intended to provide health benefits, but they may contain ingredients that can affect drug metabolism. When choosing a supplement, patients must read the label carefully to make sure it does not contain grapefruit extract, which may affect the effectiveness of bosutinib.
