Obeticholic acid drug classification and its mechanism of action
Obeticholic acid (Obeticholic acid) is an emerging drug classified as a steroid derivative, specifically a farnesoid X receptor (FXR) agonist. As a nuclear receptor, FXR is widely distributed in the liver and intestines and is involved in regulating various physiological processes such as bile acid metabolism, lipid metabolism, glucose metabolism, and inflammatory response.
Obeeticholic acid binds to FXR and activates the receptor, thereby affecting the synthesis and secretion of bile acids, and plays a key role in the treatment of biliary-related diseases such as primary biliary cholangitis (PBC). Its mechanism of action is mainly reflected in reducing the concentration of bile acids in hepatocytes, inhibiting the synthesis of cholesterol and promoting the transport of bile acids from hepatocytes to bile ducts, thereby promoting the secretion of bile. This process not only reduces the liver's exposure to bile acids, but also limits the total amount of circulating bile acids, helping to mitigate liver cell damage caused by bile acid accumulation.

In clinical application, obeticholic acid is usually used in combination with ursodeoxycholic acid (UDCA), especially for those PBC patients who have poor response to UDCA or cannot tolerate UDCA. Its safety and effectiveness have been confirmed in multiple clinical trials. Although some adverse reactions may occur during use (such as itching, fatigue and abdominal pain, etc.), the overall tolerability is good. Therefore, obeticholic acid has gradually become one of the important drugs for the treatment of PBC.
In addition, obeticholic acid has shown research potential in other diseases related to bile acid metabolism, such as non-alcoholic fatty liver disease (NAFLD) and metabolic syndrome. With in-depth research on FXR, the application fields of obeticholic acid are also constantly expanding, and its potential therapeutic value deserves further exploration.
