Trientine (Trientine) is an oral chelator specifically used for Wilson's disease, with trade names including Cufence and Syprine. It is a drug that targets abnormalities in copper metabolism and is used to provide an alternative treatment in patients who cannot tolerate the first-choice drug, penicillamine. Clinical experience with trientine is relatively limited, and its individual dosage should be adjusted based on the patient's specific response. There is no unified standard. Of note, trientine is not directly interchangeable with penicillamine and is not indicated for use in patients with cystinuria, rheumatoid arthritis, or biliary cirrhosis.

2. Indications

Trientine is mainly used in patients with hepatolenticular degeneration (Wilson's disease) who are intolerant to penicillamine. It reduces the copper load on the liver and other tissues by promoting the excretion of excess copper from the body, thereby mitigating disease progression. Trientine provides an important alternative when patients are intolerant to penicillamine or experience life-threatening side effects. However, trientine is less effective in other diseases. For example, in cystinuria, it lacks the key sulfhydryl moiety and cannot bind cystine, so it has no therapeutic effect; in studies of patients with rheumatoid arthritis, it did not show improvement in clinical or biochemical indicators after 12 weeks of treatment. 3. Usage and dosage

The dose of trientine has not been determined through systematic studies, but clinical experience shows that the initial dose is 500-750 mg per day for children and 750-1250 mg per day for adults, which can be taken in two to four divided doses. The dose may be increased to 2000 mg daily for adults and to 1500 mg for children under 12 years of age under certain circumstances.
