Can obeticholic acid treat cirrhosis caused by hepatitis B?
Obeticholic acid(Obeticholic acid) is not currently used to treat hepatitis B or hepatitis B-related cirrhosis. Its main indication is Primary biliary cholangitis (PBC), which is an autoimmune bile duct injury disease that is completely different in pathological mechanism from cirrhosis caused by viral hepatitis. OCA activates FXR receptors, regulates bile acid synthesis and excretion, and reduces inflammation and fibrosis caused by cholestasis. It is currently one of the important drugs in the treatment of PBC, especially for patients with unsatisfactory effects on ursodeoxycholic acid.

Liver cirrhosis caused by hepatitis B is a viral chronic liver disease, and its treatment goals are to inhibitHBV replication, reduce inflammatory response, and delay the progression of fibrosis. The current standard treatment for hepatitis B-related cirrhosis includes antiviral drugs (such as entecavir, tenofovir, etc.) and adjuvant treatment for liver fibrosis. Obeticholic acid lacks direct antiviral or antifibrotic effects in this disease process and is not recommended for use in patients with hepatitis B cirrhosis.
However, from the perspective of its mechanism of action, obeticholic acid’s research on regulating bile acid metabolism, anti-inflammation and anti-fibrosis is still enlightening. Some overseas research institutions are exploring its application potential in non-viral cirrhosis or metabolic liver diseases, such as non-alcoholic steatohepatitis (NASH)-related liver fibrosis, but these studies are still in the clinical trial stage and have not been formally approved.
To sum up, the therapeutic target of obeticholic acid is focused on intrahepatic bile duct damage caused by abnormal bile acid metabolism, and it has no definite effect on hepatitis B virus-related cirrhosis. If patients develop cirrhosis due to hepatitis B, they should use antiviral drugs under the guidance of a liver specialist, and regularly monitor liver function and imaging changes. The use of obeticholic acid should be strictly limited to people with PBC or specific cholestatic diseases, and it is not advisable to try alternative hepatitis B treatment options on your own.
