Cabozantinib: A Pan-Targeted TKI in Modern Oncology Practice

　　Cabozantinib is an oral multi-targeted tyrosine kinase inhibitor(TKI)distinguished by its potent inhibition of key receptors including MET,VEGFR1/2/3,RET,AXL,KIT,and FLT3.This broad inhibitory profile enables a multi-dimensional assault on tumor biology.By targeting VEGFR,it effectively suppresses angiogenesis and starves the tumor of nutrients.Concurrently,inhibition of MET and AXL disrupts signaling pathways responsible for tumor cell proliferation,survival,and metastatic dissemination.This comprehensive mechanism underpins Cabozantinib’s broad-spectrum antitumor activity and provides a robust pharmacological basis for delaying clinical resistance.

　　In the management of advanced Renal Cell Carcinoma(RCC),Cabozantinib holds a pivotal position.Clinical evidence supports its use across both first-line and subsequent therapy lines,demonstrating significant improvements in Progression-Free Survival(PFS)and Overall Survival(OS)compared to established standards.Its efficacy is particularly notable in patients presenting with bone or visceral metastases.Consequently,Cabozantinib is recognized in international consensus guidelines as a standard-of-care option for intermediate-and poor-risk RCC populations.

　　Within hepatocellular carcinoma(HCC),Cabozantinib serves as a critical second-line therapeutic option.The phase III CELESTIAL trial substantiated that Cabozantinib significantly extends survival in patients whose disease has progressed on prior sorafenib or lenvatinib therapy.Importantly,this survival benefit was consistent across diverse etiologies,including viral hepatitis and alcoholic liver disease.Furthermore,in Medullary Thyroid Cancer(MTC),Cabozantinib effectively targets RET kinase activity,leading to reduced tumor burden and lowered serum biomarkers such as calcitonin and carcinoembryonic antigen,solidifying its role in managing progressive or symptomatic disease.

　　Beyond monotherapy,Cabozantinib exhibits considerable promise in overcoming resistance and enabling combination strategies.It effectively counteracts resistance mechanisms mediated by MET amplification or AXL upregulation.For instance,in EGFR-mutant non-small cell lung cancer,combining Cabozantinib can restore sensitivity in patients who have developed resistance to osimertinib due to MET amplification.Ongoing global trials investigating Cabozantinib in combination with immune checkpoint inhibitors(ICIs)are yielding encouraging synergistic results,highlighting its expanding utility in the future landscape of solid tumor management.
