Alpelisib: From Breast Cancer to PIK3CA-Related Overgrowth Spectrum

　　Alpelisib is an oral small-molecule PI3Kαinhibitor that works by dampening the abnormally active PI3Kαsignaling pathway,thereby interfering with the growth and proliferation of affected cells.Although it is best known as a targeted cancer therapy,its use is not confined to oncology.It also has a role in certain overgrowth disorders driven by PIK3CA alterations—but it is by no means a universal treatment for ordinary lymphatic malformations,and whether it applies depends on rigorous molecular selection.

　　In Oncology:Piqray Plus Fulvestrant

　　In breast cancer,alpelisib is approved under the brand name Piqray,used in combination with fulvestrant for patients with advanced or metastatic disease that is hormone receptor(HR)-positive,HER2-negative,and carries a qualifying PIK3CA mutation.The operative word is"qualifying"—treatment requires confirmed mutation status through appropriate genomic testing,not a decision based on histology alone.

　　Beyond Oncology:Vijoice and PROS

　　Alpelisib is also approved under the brand name Vijoice for selected patients with PIK3CA-related overgrowth spectrum(PROS).PROS is an umbrella term for a group of disorders caused by somatic activating alterations in PIK3CA,leading to overgrowth and structural abnormalities in tissues such as vessels,lymphatics,and other structures.

　　Some lymphatic malformations—including microcystic lymphatic malformations—may be driven by PIK3CA pathway dysregulation.In patients who have undergone genetic testing and meet the criteria for PROS or harbor a documented PIK3CA-related alteration,alpelisib may offer meaningful benefit:some patients experience reductions in lesion volume,decreased swelling,or improvement in associated symptoms.Responses,however,vary substantially between individuals,and not every patient will achieve the same degree of relief.

　　A Note of Caution:No Self-Medication by Diagnosis Alone

　　Three factors determine whether alpelisib is worth considering:the presence of a PIK3CA-related alteration,the specific type of lesion(macrocytic versus microcystic,localized versus diffuse),and the severity of the disease and its functional impact.Starting treatment on the basis of a diagnostic label alone is unsupported by evidence and carries avoidable risk.

　　Alpelisib does have a therapeutic role in a subset of PIK3CA-related overgrowth disorders,and some associated lymphatic malformations may shrink or improve as a result.But it is not a general-purpose therapy for all lymphatic malformations;the true benefit must be judged in light of disease classification,genetic testing results,and specialist evaluation.
