(Spiolto® Respimat®) is a specific and selective anticholinergic drug that is effective in the treatment of chronic obstructive pulmonary disease (COPD).

Respimat is a quaternary ammonium derivative that has the same affinity for five types of muscarinic receptors, M1 to M5. In the human airway, it has high affinity with receptors, dissociates slowly from muscarinic M1 and M3 receptors, and can block cholinergic nerve-mediated contraction of bronchial smooth muscle for a long time. However, it dissociates from the muscarinic M2 receptor quite quickly, thus showing independent kinetic selectivity for M1 and M2 receptors. In vitro, it produced a sustained blockade of bronchoconstriction in guinea pigs, indicating selectivity for M3 receptors. Long-lasting bronchodilation experiments show that Respimat is a long-acting anticholinergic drug because it dissociates from M1 and M3 receptors very slowly.

Respimat soft mist inhaler uses the long-acting anticholinergic drug LAMA (tiotropium bromide) as the agent. The aerosol lasts longer, extending to more than 1.2 seconds. The aerosol spray speed is slow and the ideal particle content is as high as 70%, ensuring efficient deposition of the drug in the lungs, reaching 51.6%. The new device is easy to use and only requires three small movements: "turn, open and press", greatly improving patient compliance.
