赛洛多辛(Silodosin)的注意事项

Silodosin is a highly selective α-1A adrenergic receptor antagonist. Silodosin capsules are currently on the market in my country and are included in medical insurance.

Precautions before taking Silodosin

1. Clarify the indications and contraindications

(1) Applicable population: only suitable for male patients diagnosed with benign prostatic hyperplasia. It can significantly improve symptoms such as frequent urination, urgency, and difficulty in urination. Note: Not used to treat high blood pressure.

(2) Absolute contraindications: The following patients are strictly prohibited from using silodosin:

Patients with severe renal insufficiency: creatinine clearance less than 30mL/min.

Severe liver insufficiency: Child-Pugh score ≥10.

Those who are taking strong CYP3A4 inhibitors: such as ketoconazole, clarithromycin, itraconazole, ritonavir, etc.

Those who are allergic to silodosin or any ingredients in the preparation.

2. Conduct a comprehensive medical evaluation

(1) Exclude prostate cancer: BPH and prostate cancer have similar symptoms and may coexist. Before taking medication, the possibility of prostate cancer must be ruled out through digital rectal examination, PSA testing and other methods.

(2) Evaluate liver and kidney function: Kidney function (calculate creatinine clearance) and liver function need to be checked before medication to determine the appropriate starting dose.

(3) Understand the history of allergies: Ask the patient in detail about his drug allergy history.

3. Inform the doctor of personal situation

Patients should proactively inform the doctor of all their health conditions, especially all the drugs they are currently taking (including prescription drugs, over-the-counter drugs and health care products), whether they have plans for cataract surgery, and whether they have a history of hypotension or dizziness.

Precautions during the use of Silodosin

1. Strictly follow the correct usage and dosage

(1) Dosage: The usually recommended dose is 8 mg, taken once a day with a meal. In patients with moderate renal insufficiency, the dose should be halved to 4 mg once daily.

(2) How to take: It must be taken with food to reduce potential adverse reactions. If you have difficulty swallowing the capsule, carefully open the capsule, sprinkle the contents over a tablespoon of applesauce and take it immediately (within 5 minutes) without chewing, drinking a glass of cool water to ensure that all the powder is taken.

2. Be wary of potential adverse reactions

(1) Orthostatic hypotension: Dizziness, dizziness or even fainting may occur when standing up in the early stage of taking the drug. Therefore, when starting treatment or increasing the dose, you should avoid hazardous activities such as driving, operating machinery, or working at heights until you know how your body will respond to the drug.

(2) Retrograde ejaculation: This is the most common adverse reaction, which occurs when semen enters the bladder retrogradely during ejaculation instead of being discharged from the body. Although this is harmless and reversible after stopping the drug, it may affect fertility and needs to be informed in advance.

(3) Other common reactions: may include diarrhea, headache, nasal congestion or nasopharyngitis, etc.

3. Pay attention to drug interactions

(1) Avoid combined use of drugs: It is strictly prohibited to use it with strong CYP3A4 inhibitors (see above) and other α-blockers (such as terazosin, doxazosin).

(2) Drugs to be used with caution:

Antihypertensive drugs: Concurrent use may increase the risk of hypotension, and blood pressure needs to be closely monitored.

PDE5 inhibitors: such as sildenafil and tadalafil, combined with silodosin may increase the risk of orthostatic hypotension.

Powerful P-glycoprotein inhibitors: such as cyclosporine, may increase the plasma concentration of silodosin and are not recommended for combined use.

Post-medication and long-term management of Silodosin

1. Insist on regular follow-up visits

Regular follow-up visits should be carried out according to the doctor's instructions, so that the doctor can evaluate the improvement of BPH symptoms (such as IPSS score, urinary flow rate test) and check whether there are any adverse reactions.

2. Monitor efficacy and tolerance

Patients should pay attention to changes in their own symptoms, such as whether urination becomes smoother and nocturia is reduced. At the same time, pay attention to whether symptoms such as dizziness and fatigue persist or worsen, and provide timely feedback to your doctor.

3. Discontinuation of medication under special circumstances

If you plan to have surgery, especially eye surgery, you should consult your doctor whether and when you need to temporarily stop medication.
