赛洛多辛(Silodosin)的用法用量

The target of Silodosin is α1A-adrenoceptor. Patients should follow the correct usage and dosage, dosage adjustment, and strictly abide by the medication precautions.

Silodosin usage and dosage

1. Dosage

For patients with normal renal function, 8mg (i.e. one 8mg capsule) can be taken once a day.

2. Taking time

Just take it at a fixed time every day. It is recommended to take it with a meal or immediately after a meal.

By taking it with food, it can not only reduce the peak concentration of the drug in the blood, but also reduce dizziness and other discomfort caused by the drug.

3. Usage for patients with dysphagia

If the patient has difficulty swallowing the capsule, carefully open the capsule and sprinkle the internal powder on a tablespoon of cool applesauce. Swallow immediately (within 5 minutes) without chewing, then drink a glass of cold water to ensure the powder is completely taken. Do not premix for storage, nor is it recommended to subpackage capsule contents.

Principles for dose adjustment of Silodosin

1. Patients with renal insufficiency

(1), moderate renal insufficiency (creatinine clearance CCr30-50mL/min): the dose should be reduced to 4 mg, once a day. Because the drug concentration in the body of such patients can reach 3 times that of normal people.

(2) Severe renal insufficiency (CCr<30mL/min): use is prohibited.

2. Patients with hepatic insufficiency

(1) Mild to moderate hepatic insufficiency: No dose adjustment is required.

(2) Severe hepatic insufficiency (Child-Pugh score ≥ 10): Use is prohibited due to lack of relevant safety studies.

3. Elderly patients

Age itself is not the basis for dose adjustment, and standard doses can be followed. Elderly patients are more sensitive to orthostatic hypotension, so necessary observation and monitoring should be strengthened during the initial medication phase.

Key Medication Precautions for Silodosin

1. Be wary of orthostatic hypotension

Hypotension symptoms such as dizziness when standing and darkening of the eyes may occur in the early stages of medication. Therefore, you should avoid driving, operating machinery, or working at heights when starting treatment or increasing your dose until your body's response to this drug is known.

2. It is prohibited to take it with specific drugs

(1) Strong CYP3A4 inhibitors: such as ketoconazole, clarithromycin, itraconazole, and ritonavir. Combined use will significantly increase the plasma concentration of silodosin and is an absolute contraindication.

(2) Other α-blockers: such as tamsulosin and doxazosin. Combined use will have additive antihypertensive effects and increase the risk of hypotension.

3. Be sure to inform your medication history before surgery

Patients who plan to undergo cataract surgery must inform their ophthalmologist in advance that they are taking or have taken silodosin. Because this drug may increase the risk of intraoperative floppy iris syndrome (IFIS), doctors need to make surgical plans in advance.
