非那雄胺可以长期服用吗?
It is a 4-nitrosteroid hormone compound and a specific type II 5α-reductase inhibitor. It can be used for BPH patients with enlarged prostate and lower urinary tract symptoms, as well as patients with larger prostate volume (≥40ml) and/or higher serum PSA levels (PSA≥1.4ng/ml) with better results.
Finasteride is taken orally, 5 mg/time, once/day. 5mg/day, can be taken in the morning and evening, before or after meals, long-term use is recommended. Patients with Renal Impairment: No dose adjustment is required in patients with various degrees of renal impairment (creatinine clearance as low as 9 mL/min). Elderly: Pharmacokinetic studies in patients over 70 years of age have shown that the clearance of finasteride is reduced, but no dose adjustment is required. When finasteride is used to treat hair loss, 1 mg is taken orally, once a day, and a course of treatment is 4 months.
No dose adjustment is required in patients with renal impairment. In patients with chronic renal dysfunction (creatinine clearance in the range of 9-55 ml/min), the AUC, Cmax and t1/2 after a single dose of 14C-finasteride are no different from healthy volunteers. Some metabolites that are normally excreted by the kidneys are excreted in the feces. Thus there is an increase in fecal excretion of metabolites and a corresponding decrease in urinary excretion.
Finasteride (5 mg) is taken orally as a single dose, and its bioavailability is 63% (34-108%), and its bioavailability is not affected by food. The plasma concentration reaches the peak 1 to 2 hours after taking the drug, and the Cmax is 37ng/mL (range: 27 to 49ng/mL). Finasteride is mainly metabolized in the liver by cytochrome P450 enzyme 3A4, and its two main metabolites play only a small role in the inhibitory activity of finasteride on 5α-reductase.
Can finasteride be taken long-term?
Whether long-term use is required requires consultation with the attending physician, who will formulate a medication plan based on the patient's own situation.
