阿普斯特片治疗效果如何？

(Apremilast) is a phosphodiesterase-4 (PDE-4) inhibitor developed by the American company Celgene in 2014 and marketed in the United States. By regulating the expression of PDE-4 in immune cells and increasing adenosine phosphate, it achieves immunity and suppresses inflammation. It is the first oral drug approved by the FDA for the treatment of plaque psoriasis (psoriasis) and arthritis. So, how effective is Apremilast?

Mechanism of action of Apremilast tablets

Apremilast is a novel oral, small molecule phosphodiesterase 4 (PDE4) inhibitor. Apremilast can inhibit PDE4 activity, increase intracellular cyclic adenosine monophosphate (cAMP) levels, further regulate the expression of tumor necrosis factor (TNF-α), IL-23 and other inflammatory cytokines, and ultimately inhibit the inflammatory response.

How effective is Apremilast tablets?

Objective: To evaluate the effectiveness and safety of apremilast in the treatment of psoriatic arthritis (PsA).

Research methods: Search relevant published studies in "PubMed", "Cochrane Library", and "OVID", and query "clinictrials.gov". Carry out literature quality evaluation and data extraction, and use Rev Man 5.3.5 software to conduct Meta analysis.

Research results: A total of 6 randomized controlled trial (RCT) documents were included, with a total of 1 645 PsA patients. Meta-analysis results show that in terms of effectiveness in treating PsA, after 12 or 16 weeks of treatment, the ACR20 remission rate of each dose combination of Apremilast was higher than that of the placebo control group, RR=1.67; HAQ-DI was also better than the placebo control group, WMD=-0.14. In terms of safety, the incidence of adverse events (AEs) in each dose group of Apremilast was higher than that in the placebo control group, RR=1.91.

Conclusion: Apremilast treatment can effectively delay or improve the condition of PsA. Although it causes adverse reactions, it is well tolerated by patients.
