How to use pulmozyme (alfa deoxyribonuclease inhaler)?

It is a recombinant human deoxyribonuclease that can hydrolyze DNA in the fluid of cystic fibrosis patients and reduce the viscoelasticity of the fluid to prevent viscous secretions from being retained in the airways, leading to decreased lung function and worsening of infection.

How to use pulmozyme (Alfa deoxyribonuclease inhaler)

1. Before using pulmozyme ampoules and nebulizers, you need to wash your hands thoroughly with soap and water and clean the flat table to help prevent infection.

2. With the atomizer components within easy reach on a clean, flat table, test the compressor by turning it on and placing your finger in front of the "air output" or "air" port to feel the air flow.

3. Take out a bag of Pulmozyme foil bags from the refrigerator. Open the foil bag, remove 1 ampoule of Pulmozyme, return the remaining ampoule to the foil bag, and return to the refrigerator. Check the expiry date printed on the packaging; do not use it if it is expired.

4. Turn the ampoule upside down and squeeze it gently to check whether the ampoule leaks. If the ampoule leaks, do not use it. Also check the Pulmozyme liquid in the ampoule to ensure it is clear and free of particles. Do not use if the liquid is cloudy or discolored.

5. Connect the long connecting tube to the "air output" or "air" port on the compressor.

6. Push the mouthpiece into one end of the wider plastic T. Connect the flexible aerosol tubing to the other end of the T.

7. Unscrew or remove the cap from the atomizer cup, place the atomizer cup face up on the table, and place the cap upside down on a clean surface.

8. Hold the tab at the bottom of the pulmozyme ampoule firmly and twist off the top without squeezing the ampoule.

9. Turn the ampoule upside down and squeeze gently, pour the medicine into the atomizer cup, continue to squeeze until the ampoule is empty, and screw or snap the lid onto the atomizer cup.

10. Connect the plastic T to the atomizer cap. If you are using a Sidestream atomizer, please connect the mouthpiece to the top of the atomizer. If you are using a PARIBABY atomizer, please connect the elbow and mask to the atomizer outlet.

11. Push upwards firmly to connect the open end of the long connecting tube to the port at the bottom of the atomizer cup.

12. Turn on the compressor and check if there is any mist coming out of the atomizer.

13. Place the mouthpiece between your teeth and over your tongue, and slowly inhale and exhale through your mouth without blocking the airflow with your tongue. Do not breathe through your nose, use a nose clip if you have difficulty breathing through your mouth alone. When the atomizer starts to spit out, tap the atomizer cup and continue breathing until the atomizer cup becomes empty or no more mist is produced. If you need to stop treatment before it ends, or if you start to cough, turn off the compressor and do not spill any medicine.

Therapeutic effects of pulmozyme

In a randomized, placebo-controlled trial of patients with clinically stable cystic fibrosis who received placebo or pulmozyme, pulmozyme significantly reduced the number of patients with respiratory infections requiring parenteral antibiotics compared with placebo, with a relative risk reduction of 27% to 29%.

Within 8 days of starting treatment with pulmozyme, the average FEV1 of the treatment group increased by 7.9%-9.0%, showing better efficacy compared with placebo.

Common symptoms of pulmozyme include voice changes, pharyngitis, rash, laryngitis, chest pain, conjunctivitis, rhinitis, reduced vital capacity, fever, indigestion, dyspnea, etc.
