Pomalidomide Side Effects, Risk Monitoring, and Precautions
Pomalidomide can cause blood cell reductions, infection, fatigue, gastrointestinal reactions, and blood clots.Severity varies with the individual patient and the combination regimen, with neutropenia, serious infection, and thrombotic risk drawing particular attention.
1.Common side effects
In patients with multiple myeloma(MM)or Kaposi sarcoma(KS),commonly reported reactions include fatigue, weakness, neutropenia, anemia, constipation, nausea, diarrhea, shortness of breath, upper respiratory infection, and fever. Some of these symptoms may be related to co-administered dexamethasone or other anticancer agents.
2.Potentially serious side effects
Neutropenia can raise the risk of infection, and severe anemia or thrombocytopenia may interfere with treatment. The drug has also been associated with deep vein thrombosis, pulmonary embolism, myocardial infarction, and stroke.Liver injury, severe skin reactions, and allergic reactions may also occur and warrant close monitoring.
3.Symptoms that need prompt attention
Fever, chills, unusual bleeding,or marked weakness should prompt evaluation of blood counts and infection status.Chest pain, sudden shortness of breath, swelling in one leg,or abrupt neurological symptoms may signal a thrombotic event.Facial swelling, difficulty breathing,or a severe rash also requires immediate medical assessment.
4.Reducing the risk from side effects
Regular monitoring of blood counts and liver function is usually required during treatment, along with assessment of thrombotic risk based on the clinical situation. Some patients need antithrombotic prophylaxis, with the approach tailored to individual risk and concomitant medications.Severe adverse reactions may require treatment interruption or dose adjustment.
5.Special precautions
Pomalidomide carries a serious risk of embryo-fetal toxicity and is contraindicated during pregnancy.Patients who can become pregnant must follow the required pregnancy testing and contraception measures and use the medicine under its risk management program.
