(febuxostat) is a new anti-gout drug approved by the US FDA in February 2009 for the control of hyperuricemia in gout patients. So, if patients with gout want to benefit from this drug while taking it, should they take febuxostat before or after meals?

The answer is yes. Febuxostat is not affected by food, so patients can take it before or after meals, and patients can make a decision based on their actual situation. Febuxostat is an inhibitor of xanthine oxidase and can increase the blood concentration of drugs metabolized by xanthine oxidase, such as theophylline and azathioprine. Therefore, the above drugs should be avoided when taking febuxostat. When they must be used, the dosage of the above-mentioned drugs should be reduced and their blood drug concentrations in the body should be measured regularly to avoid serious adverse reactions. Antacids will affect the absorption rate of febuxostat but not its absorption, so there is no need to increase the dosage when taking them at the same time.

In terms of its mechanism of action, Febuxostat (Febuxostat) has two important characteristics: (1) Febuxostat can simultaneously and efficiently inhibit the oxidized and reduced forms of xanthine oxidase. (2) Febuxostat is highly selective for xanthine oxidase. Febuxostat has no obvious activity on other purine and pyrimidine metabolism enzymes. Therefore, compared with allopurinol, febuxostat not only has a smaller dosage, but also does not affect other pyrimidine metabolism enzymes except xanthine oxidase, which is an important reason for the toxic effects of allopurinol.

Although the drug has good efficacy, patients still need to pay attention during the actual use of febuxostat: patients who cannot tolerate the serious adverse reactions caused by febuxostat, and patients with moderate to severe alcoholic liver damage (transpeptidase > 3 times the upper limit of normal or Pugh score in children > 9) need to be disabled (febuxostat), otherwise it may lead to the occurrence of some serious adverse events, thereby affecting the patient's therapeutic effect.
