The therapeutic effect of febuxostat in the treatment of gout:

This study compared the efficacy and safety of febuxostat and allopurinol in reducing serum uric acid (SUA) levels in patients with gout (GOUT).

Methods 486 gout patients were randomly divided into experimental groups: febuxostat 40 mg group (40 mg, once a day), febuxostat 80 mg group (80 mg, once a day) and allopurinol control group (100 mg, 3 times a day). The treatment course was 24 weeks.

The primary efficacy indicator is the proportion of patients whose blood uric acid levels have dropped below 360 μmol/L at the end of 24 weeks. The secondary efficacy indicators are the proportion of patients whose blood uric acid levels have dropped below 360 μmol/L at the end of 24 weeks, the level of blood uric acid decline before and after 24 weeks of treatment, and the number of gout attacks.

Results: The intention-to-treat analysis set (ITT) analysis of subjects in the febuxostat 40 mg group, febuxostat 80 mg group, and allopurinol group at the last visit with SUA ≤ 360 μmol/L was 49.06%, 65.38%, and 43.67%, respectively. There was a statistically significant difference in the compliance rates among the three groups with subjects whose SUA was reduced to less than 360 mol/L 24 weeks after treatment (P0.05). There was no statistically significant difference in the overall incidence of adverse reactions among the three groups.

Conclusion: The 40 mg group is equivalent to the allopurinol group; the febuxostat 80 mg group is superior to the allopurinol group, and is also superior to the febuxostat 40 mg group. Febuxostat is well tolerated for long-term use in lowering uric acid.
