非布司他怎么使用？

The tablet is a new, highly effective, non-purine selective inhibitor of xanthine oxidase. It reduces blood uric acid concentration by inhibiting the production of uric acid, and is suitable for the treatment of hyperuricemia in patients with gout. So, how to use febuxostat?

The therapeutic dose of febuxostat starts at 40 mg daily, and after two weeks, the recommended dose is 80 mg daily for patients whose serum uric acid levels are still higher than 6 mg per liter.

When used to treat hyperuricemia patients with gout symptoms, the recommended dose of this product is 40 mg or 80 mg once daily. There is no need to consider the effects of food or antacids when taking febuxostat.

Generally, taking febuxostat for 6 months can remove excess blood uric acid from the body. After 6 months of treatment:

(1) If the serum uric acid level remains between 240 umol/l and 360 umol/l, maintain febuxostat 40 mg/day treatment. 

(2) If the blood uric acid level is higher than 360 umol/l, increase the dose of fexostatin by 60 mg/day, up to a maximum of 80 mg/day, and monitor blood uric acid, liver and kidney function regularly. 

(3) If the blood uric acid level is lower than 240 umol/l, the dose of febuxostat can be reduced to 20 mg/day and the treatment can be maintained for 6 months.

In addition, the 2016 edition of the "China Gout Diagnosis and Treatment Guidelines" believes that reducing blood uric acid to below 360 μmol/L can control the disease, promote the dissolution of existing tophi, and prevent the occurrence of kidney disease.

The uric acid-lowering effects of different doses of febuxostat have been compared. After 6 weeks of treatment, 10 mg/day of febuxostat can reduce blood uric acid by 20.1%; 20 mg/day of febuxostat can reduce blood uric acid by 31.5%; 40 mg/day of febuxostat can reduce blood uric acid by 41.9%; fluctuations in blood uric acid levels can easily induce acute attacks of gout.

To prevent gout attacks during the initial period of use, it is recommended to take NSAIDs or colchicine at the same time. Preventive treatment is maintained for 3 to 6 months, and is adjusted according to the patient's gouty arthritis attack.
