The recommended oral dose of febuxostat is 40 mg or 80 mg once daily. The recommended starting dose of this product is 40 mg once daily. After a dose of 40 mg for two weeks, for patients whose serum uric acid level (sUA) is still higher than 6 mg/dl, the recommended dose is 80 mg.

Febuxostat is a 2-arylthiazole derivative and a xanthine oxidase inhibitor that reduces serum uric acid concentration by inhibiting uric acid synthesis. Febuxostat does not inhibit other enzymes involved in purine and pyrimidine synthesis and metabolism at conventional therapeutic concentrations. Administration of febuxostat to healthy subjects resulted in a dose-dependent decrease in the mean 24-hour serum uric acid concentration and an increase in the mean serum concentration of xanthine. In addition, the total daily excretion of uric acid is reduced. At the same time, total daily urinary excretion of xanthines increases. Febuxostat at daily doses of 40 mg and 80 mg reduced mean 24-hour serum uric acid concentrations by 40% to 55%. Tests were conducted on healthy subjects and gout patients respectively, and the effect of this product on cardiac repolarization was evaluated by measuring the QTc interval. This product has a daily dose of 300 mg and has no effect on the QTc interval at steady state.

Febuxostat is one of the commonly used uric acid-lowering drugs in gout clinical practice. Compared with other drugs, Febuxostat is safe and powerful. However, from the perspective of the mechanism of febuxostat lowering uric acid, it is still a "symptomatic" treatment, which only acts downstream of the mechanism of increasing uric acid and does not fundamentally solve the cause of increased uric acid. In other words, despite the application of febuxostat, the cause of hyperuricemia has not been eradicated, and long-term or even lifelong use of febuxostat is required to maintain blood uric acid levels at target.

It can be seen that although febuxostat is relatively safe and requires long-term use, its related side effects are still an important factor that troubles gout patients. In order to use febuxostat more safely, many patients have been thinking about the dosage and usage. Pharmaceutical companies and local gout guidelines have relevant recommendations. Simply put, the safest dosage is to use the smallest dose that suits you to maintain your blood uric acid stably and up to standard.
