What are the precautions for using canakinumab?

(Canakinumab) is a human monoclonal antibody that can selectively inhibit IL-1β, significantly reduce systemic CRP and other inflammatory marker levels, and is well tolerated. It is currently mainly used to treat inflammatory diseases caused by hereditary IL-1β, such as Muckle-Wells syndrome. A study has enrolled 17,200 patients with stable myocardial infarction and persistently elevated hsCRP levels. They were randomly divided into canakinumab treatment groups and control groups. Precautions when using canakinumab

1. Simultaneous use of TNF-alpha or IL-1 antagonists.

2. If serious infection occurs, please interrupt treatment.

3. May increase the risk of lymphoma.

4. May weaken the defense against malignant tumors.

5. Macrophage Activation Syndrome (MAS) is a known life-threatening disease that may develop with rheumatic diseases, especially SJIA. According to clinical trials, canakinumab does not increase the incidence of MAS.

6. In individual cases, unusual or opportunistic infections include aspergillosis, atypical mycobacterial infections, cytomegalovirus, herpes zoster; mainly upper respiratory tract infections, and in some cases serious infections. A causal relationship between treatment and infection cannot be excluded.
