What is Vigabatrin (Okeda) and its application scope in neurology
Vigabatrin (Vigabatrin) is a central nervous system drug that is a γ-aminobutyric acid (GABA) aminotransferase inhibitor. It irreversibly inhibits GABA transaminase, reduces the decomposition of GABA, and increases the concentration of the inhibitory neurotransmitter GABA in the brain, thus enhancing the neuroinhibitory effect and playing an anti-epileptic effect. Different from traditional anti-epileptic drugs, Vigabatrin has a more unique mechanism of action and therefore shows special efficacy in some patients with refractory epilepsy.
In terms of clinical application, Vigabatrin is mainly used for the treatment of refractory partial epilepsy and infantile spasms (also known as West syndrome). Vigabatrin is often used as a second-line or auxiliary medication for epilepsy patients whose epilepsy is poorly controlled by conventional medications. For example, some scholars have tried to apply it to certain diseases related to abnormalities in the GABAergic system, such as psychomotor disorders and neurodegenerative diseases, but these uses have not yet reached a mature clinical consensus. Current mainstream guidelines still position vigabatrin in the management of refractory epilepsy and infantile spasms.
It should be noted that safety issues with Vigabatrin limit its widespread use. Long-term use may cause visual field defects and irreversible retinal damage, so patients need regular eye examinations during treatment. In addition, side effects such as drowsiness, dizziness, weight gain, and abnormal mental behavior may also occur. Taken together, Vigabatrin is a highly targeted anti-epileptic drug in neurology and has outstanding effects on specific groups of people, but it must be used rationally under strict monitoring.
