The Difference Between Etrasimod and Mesalazine
Etrasimod and mesalazine are both drugs used to treat ulcerative colitis (UC), but their mechanisms of action and clinical applications are different.
Mesalamine is a 5-aminosalicylic acid (5-ASA) drug that mainly relieves the symptoms of ulcerative colitis by locally inhibiting the inflammatory response in the colon. The mechanism of action of mesalamine is mainly by directly inhibiting inflammatory factors in the intestine, such as tumor necrosis factor (TNF-α), interleukins (IL-1 and IL-6), etc., reducing damage to the intestinal mucosa, thereby alleviating patients' symptoms such as diarrhea, bloody stools, and abdominal pain.

Unlike mesalazine, itridimod acts as a sphingosine-1-phosphate (S1P) receptor modulator, reducing inflammation by modulating the function of the immune system. Itramod inhibits the migration of immune cells (especially T cells) in the intestine by binding to the S1P receptor, thereby reducing the release of inflammatory factors and alleviating intestinal immune responses and symptoms. Compared with mesalazine, itramod has a more systemic effect, not only acting in the local intestine, but also regulating the immune response throughout the body.
In terms of therapeutic effect, mesalazine is mainly suitable for patients with mild to moderate ulcerative colitis, and its effect is limited to relieving symptoms. Itramod is more suitable for patients with moderately to severely active ulcerative colitis, especially those who have not responded well to traditional treatments. The systemic immunomodulatory effects of itrimod enable it to provide a broader range of therapeutic effects.
Overall, itridimod and mesalazine have different mechanisms of action and indications in the treatment of ulcerative colitis. Mesalazine mainly inhibits intestinal inflammation locally, while itramod alleviates disease by modulating systemic immune responses.
