Are bedaquiline's side effects serious? Do you need to pay attention to the risks of taking the drug?
Bedaquiline (Bedaquiline) is a new anti-tuberculosis drug, mainly used to treat multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). It belongs to the ATP synthetase inhibitor class of drugs. Due to its unique mechanism of action, bedaquiline has high efficacy in the treatment of drug-resistant tuberculosis. However, its efficacy is accompanied by a certain risk of side effects. One of the most concerning adverse reactions of bedaquiline is cardiotoxicity, especially QT interval prolongation, which may lead to irregular heartbeats and even serious arrhythmias. The risk is further increased especially when bedaquiline is combined with other drugs that also prolong the QT interval (e.g., moxifloxacin, fluoroquinolones). Therefore, ECG monitoring must be carried out regularly during medication, especially in the 2nd and 12th weeks, in order to detect changes in the QT interval in a timely manner and adjust the treatment plan.

Bedaquiline may also have certain effects on liver function. Some patients experienced elevated liver enzymes while taking the drug, and even developed severe liver damage in individual cases. Therefore, it is recommended to evaluate the basic status of liver function before taking medication and to check liver function indicators regularly during treatment. For patients with existing liver disease or concomitant use of other hepatotoxic drugs, bedaquiline should be used with greater caution and consideration should be given to adjusting the dose or changing the treatment regimen if necessary.
Bedaquiline may also cause minor but quality-of-life side effects such as headache, joint pain, nausea, decreased appetite, and rash. Some patients report fatigue or sleep disturbance after taking the drug, and most of these reactions can be relieved by continued treatment or appropriate intervention. Generally speaking, the side effects of bedaquiline are controllable in most patients, but considering that it is used in high-risk patient groups, it is recommended to use it rationally under the guidance of tuberculosis specialists, and to establish a complete monitoring mechanism to ensure that both treatment efficacy and drug safety are paid equal attention to.
