What is the standard daily dose of imatinib?
Imatinib/Gleevec , as an oral small molecule tyrosine kinase inhibitor (TKI), has clear dosage specifications in the treatment of chronic myelogenous leukemia (CML) and gastrointestinal stromal tumor (GIST). For CML patients, the recommended standard starting dose is 400 mg orally once daily, which has been shown to effectively inhibit the activity of the BCR-ABL fusion protein and induce hematological and molecular remission in patients. In some patients in the accelerated phase or blast phase, doctors may increase the dose to 600mg or even 800mg/day, but this needs to be judged based on the severity of the disease, patient tolerance and toxic side effects.

For patients with gastrointestinal stromal tumors (GIST), the standard starting dose also recommended is 400 mg once daily. This dose can effectively target KIT or PDGFRA mutations, thereby inhibiting tumor cell growth. If some GIST patients are resistant to standard doses or relapse, clinical considerations will also be given to increasing the dose to 600-800 mg, but this needs to be done under close monitoring to ensure that adverse reactions are controllable.
It is recommended to take imatinib orally with or after meals, along with plenty of water (a full cup is recommended) to reduce the occurrence of side effects such as gastrointestinal discomfort. In clinical practice, it has been found that regular medication can significantly improve the efficacy and reduce adverse reactions. Therefore, it is recommended that patients take it at the same time every day to avoid missing doses or doubling the dosage.
If the patient experiences obvious adverse reactions during treatment, such as rash, diarrhea, bone marrow suppression, etc., the doctor may temporarily adjust the dose or suspend the treatment, and then gradually resume the original dose after the symptoms are relieved. The blood concentration of imatinib has an important impact on the efficacy. If it is too low, it may lead to a decrease in efficacy, and if it is too high, it may increase side effects. Therefore, rational use of the dose is particularly critical.
