Key points on dosage and usage during Pegloticase medication
Pegloticase is a drug used to treat refractory chronic gout, especially for patients who have poor efficacy or are intolerant to other urate-lowering drugs (such as allopurinol or febuxostat). The drug is a PEGylated recombinant urate oxidase that can oxidize uric acid into allantoin that is more soluble in water, thereby rapidly and significantly reducing blood uric acid levels.
The recommended dose of Pegloticase is an intravenous infusion of 8 mg once every two weeks, with each infusion lasting no less than 2 hours. The drug must be diluted and not given as a bolus or rapid infusion. Before use, the drug needs to be diluted in 100 mL of 0.9% sodium chloride injection and used within 4 hours after preparation.

Before using this product, it is recommended that patients stop taking other urate-lowering drugs (such as allopurinol) to avoid interfering with the efficacy monitoring of Pegloticase. In addition, it is recommended to regularly detect blood uric acid levels before the start of treatment and during treatment. If the uric acid value is 6 mg/dL twice in a row, it indicates that the patient may have developed drug resistance. At this time, it is recommended to stop Pegloticase treatment to prevent allergic reactions or weakening of the efficacy.
To reduce the risk of allergic reactions, pretreatment with antihistamines and corticosteroids is recommended before each infusion. This drug is contraindicated in patients with a history of G6PD deficiency to prevent hemolysis or methemoglobinemia.
This product is a long-term treatment plan, and the length of treatment depends on the patient's condition and treatment response. In some patients, after several months of treatment, gout nodules can significantly shrink or disappear, and joint function can also improve.
