Analysis of serious side effects and contraindications of pirfenidone
Although Pirfenidone is recognized as one of the basic drugs for the treatment of idiopathic pulmonary fibrosis (IPF), its adverse reactions and contraindications still require great attention in actual clinical use, especially in the context of long-term medication. The most common side effects include gastrointestinal reactions such as nausea, loss of appetite, diarrhea, indigestion, etc. Most of these appear in the early stages of treatment and can be alleviated by taking the medicine with meals or adjusting the diet structure. In addition, skin photosensitivity is also a significant problem. Some patients may develop rashes, erythema and even phototoxic blisters after being exposed to the sun. Therefore, strict sun protection should be used during medication to avoid sun exposure.

Of greater concern is abnormal liver function, which is one of the serious side effects of pirfenidone use. Drug metabolism mainly depends on the liver. If long-term medication causes liver enzymes such as ALT and AST to increase, liver function indicators need to be monitored regularly, and treatment should be suspended or reduced if necessary. Other rare but serious side effects include high blood pressure, dizziness, fatigue and worsening of cough. Pirfenidone should be used with caution or even contraindicated in patients with moderate to severe hepatic impairment, severe renal insufficiency, or those who have received organ transplants. There is currently insufficient data to support use in children, pregnant women, and lactating women and should be avoided.
In addition, pirfenidone has potential drug interactions with certain drugs. For example, potentCYP1A2 inhibitors (such as fluvoxamine) may increase their plasma concentration, leading to enhanced toxicity. Careful evaluation is required when using combined drugs. Smoking also affects drug metabolism, and patients are advised to quit smoking during treatment. In order to achieve a safer medication experience, it is recommended that patients receive a comprehensive assessment before starting treatment and have regular follow-up monitoring of liver function, skin status and systemic adverse reactions.
Generally speaking, although pirfenidone has clear efficacy, it should be used under the guidance of a doctor throughout the entire process, and attention should be paid to side effect prevention and individualized adjustment to maximize the efficacy and safety.
