Can selumetinib achieve complete removal effect in treating café-au-lait spots?

Selumetinib is a selective MEK inhibitor, mainly used to target and inhibit the RAS-RAF-MEK-ERK signaling pathway, which is abnormally activated in many tumors and genetic diseases. In recent years, selumetinib has attracted widespread attention in the treatment of neurofibromatosis (NF1)-related cutaneous manifestations, especially café-au-lait macules (CALMs). Although selumetinib has shown some potential in reducing skin lesions and improving symptoms, whether it can achieve complete removal of café-au-lait spots still requires careful analysis.

Café-au-lait spots, the most common skin manifestation in patients with neurofibromatosis, are formed due to abnormal proliferation and pigmentation of melanocytes in the skin. Selumetinib can regulate cell proliferation and differentiation processes by inhibiting the abnormally active MEK pathway, and has a certain inhibitory effect on the activity of pigment cells in diseased skin. In some clinical observations, patients taking selumetinib showed a reduction in the color of café-au-lait spots and an improvement in skin texture.

Current clinical evidence shows that selumetinib cannot completely “eradicate” café-au-lait spots. The pigmentation of café-au-lait spots is relatively deep and structurally complex, and MEK inhibitors are more effective at controlling and alleviating symptoms rather than completely eliminating the spots. Partial degradation of pigment may require longer treatment and follow-up, and individual differences are large, so some patients may have limited efficacy.
In summary, selumetinib has a certain auxiliary effect in the treatment of café-au-lait spots and can improve the degree and appearance of the spots, but it cannot guarantee the complete removal of café-au-lait spots. In the future, more long-term, large-scale clinical studies are needed to evaluate the sustainability and safety of its efficacy. At the same time, combined with other treatment methods, exploring multi-modal combination therapy may bring more ideal treatment effects to patients.
