Why do many doctors not recommend Guanfacine?
Guanfacine is widely used in the treatment of attention deficit hyperactivity disorder (ADHD) and other neuropsychiatric diseases abroad. However, in clinical practice, some doctors are still cautious about its use. The main reasons involve the following aspects. First, the onset of effect is slow. Compared with traditional central nervous system stimulants such as methylphenidate or amphetamines, guanfacine has a relatively mild mechanism of action and usually requires continuous administration for several days to weeks to take effect. Therefore, when acute behavioral problems or attention deficits are significant, its short-term control effect is not as rapid and obvious as stimulant drugs.
Secondly, the common side effects are relatively obvious. Common adverse reactions of guanfacine include sedation, drowsiness, decreased blood pressure, dizziness, and bradycardia. Significant daytime drowsiness or attention lag may occur, especially in children and adolescents. Although most of these side effects can be alleviated or controlled after adjusting the dose during long-term use, they may interfere with daily study and life during the initial treatment stage, affecting patient compliance.
Additionally, some doctors remain on the fence about its long-term safety. Especially in the context that it has not yet been officially launched in mainland China, doctors have relatively little experience with its pharmacokinetics, interactions with other drugs, and individualized medication adjustments. This makes clinical practice more inclined to choose familiar treatment options that are already on the market, such as methylphenidates or atomoxetine.
In summary, although guanfacine has unique advantages as a non-stimulant ADHD treatment drug, many doctors still prefer to recommend more mature and quickly effective treatment options due to factors such as slow onset of action, side effects such as drowsiness, and lack of official marketing in China.
