What neurological diseases is Opicapone mainly used to treat?
Opicapone is a selective and reversible catechol-O-methyltransferase (COMT) inhibitor. It is mainly suitable for the treatment of patients with Parkinson’s disease (Parkinson’s Disease), especially those who experience phasic movement fluctuations after treatment with levodopa (Levodopa). This situation is more common in patients with Parkinson's disease after long-term use of levodopa. It is characterized by rapid drug effect fading, and variable mobility, which affects the quality of daily life. Opicapone was introduced into the treatment system precisely to solve the problems caused by fluctuations in levodopa blood concentration.

Clinically, opicapone is often used as an auxiliary drug in the combined regimen of levodopa/dopa decarboxylase inhibitors (such as carbidopa). It does not itself have dopamine-like activity. Instead, it inhibits COMT-mediated peripheral metabolism of levodopa and prolongs the half-life of levodopa in the blood, thereby stabilizing its concentration in the brain and improving the therapeutic effect. It is suitable for patients with advanced Parkinson's disease, especially those who still have "end-of-dose phenomenon" or fluctuations in motor symptoms after taking levodopa.
In addition to Parkinson's disease, Opicapone has not yet been approved for indications in other neurological diseases, but in some studies, it is also being explored for use in movement disorders, tremors and other dopamine-related pathological conditions. It is worth noting that opicapone is different from traditional COMT inhibitors (such as entacapone and tolcapone) in that its administration frequency is lower and its side effects are milder, which helps improve patient compliance. As such, it has become an increasingly important component of the long-term management of Parkinson's disease.
