What are the common side effects of neratinib/neratinib?
Neratinib is an oral tyrosine kinase inhibitor mainly used to treat HER2-positive breast cancer. Although it has shown efficacy in extended adjuvant therapy, its side effects, especially gastrointestinal effects, require special attention.
The most common side effect is diarrhea, which nearly all patients experience to varying degrees. Research shows that about 95% of patients experience diarrhea in the early stages of treatment, and about 40% of them experience severe diarrhea, which may lead to dehydration, electrolyte imbalance, and even kidney function damage. Therefore, prophylactic use of antidiarrheal drugs, such as loperamide, is recommended at the beginning of treatment to reduce symptoms.

In addition to diarrhea, other common side effects include nausea, vomiting, fatigue, abdominal pain, rash, decreased appetite, stomatitis, and muscle cramps. These symptoms usually appear early in treatment and may improve over time. Some patients may also have abnormal liver function, manifested by elevated liver enzymes, and liver function indicators need to be monitored regularly.
In addition, neratinib may cause skin reactions, such as rash and dryness, and an increased risk of urinary tract infections. Some patients report symptoms such as weight loss, headaches, joint pain, and dizziness. Although most of these side effects are mild to moderate, close observation is required and treatment options should be adjusted based on symptoms.
While taking neratinib, patients should have regular blood tests to monitor liver function and electrolyte levels. For patients who experience serious side effects, dose adjustments or temporary discontinuation of treatment may be necessary. Doctors should develop individualized management plans based on the patient's specific conditions to ensure the safety and effectiveness of treatment.
