Is tofacitinib mainly used to relieve pain or treat immune diseases?
Tofacitinib(Tofacitinib) is not an analgesic drug in the traditional sense. It is a small molecule targeted oral drug mainly used to treat a variety of chronic immune-mediated diseases. This drug regulates and inhibits the expression of multiple pro-inflammatory factors in the immune system by inhibiting the Janus kinase (JAK) signaling pathway. It is a typical immunomodulator. Although clinically, tofacitinib can effectively relieve symptoms such as joint swelling, pain, and morning stiffness, these effects are achieved by inhibiting immune-mediated inflammatory responses rather than directly acting on the pain transmission pathways of the nervous system. Therefore, it does not have the same analgesic mechanism as nonsteroidal anti-inflammatory drugs (NSAIDs) or opioid analgesics.

Tofacitinib is mainly used to treat chronic inflammatory diseases such as moderately to severely active rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis. It is especially suitable for patients who have poor response or intolerance to traditional DMARDs (such as methotrexate). By selectively inhibiting JAK1 and JAK3, tofacitinib interferes with the transmission of inflammatory signals such as interleukins and interferons, fundamentally blocking the inflammatory cascade and helping to control disease progression and tissue damage caused by immune attacks. Many clinical studies have shown that tofacitinib is as effective as anti-TNF-α monoclonal antibodies and other biological agents in improving disease activity, but it has the advantages of oral administration, rapid onset of action, short drug half-life, and strong controllability.
It is important to note that although tofacitinib significantly improves pain symptoms, in the pain management field it is not indicated for the treatment of pain due to non-inflammatory causes such as neuropathic, neoplastic, or surgical. Because it works by regulating immune pathways, you need to pay attention to risks related to immunosuppression, such as infection, herpes zoster, elevated blood lipids, abnormal liver function, etc. when using it.
