How effective is Trientine Hydrochloride Capsules in treating heart failure? Is it recommended for patients with heart failure?
Trientine hydrochloride (Trientine ) is a copper chelator traditionally used to treat Wilson's disease. In recent years, it has also been studied for the treatment of specific types of heart failure (HF), especially cardiomyopathy related to copper metabolism disorders. In some patients with heart failure, abnormal copper metabolism may be related to myocardial structural damage and decreased cardiac function. Therefore, using trientine to regulate copper levels is expected to improve the cardiac function of these patients.
Some small-scale clinical studies have shown that trientine may have certain efficacy in patients with heart failure with preserved ejection fraction (HFpEF). This type of heart failure is usually associated with old age, hypertension, diabetes, and metabolic syndrome. Patients often present with decreased cardiac compliance and diastolic dysfunction. Studies have found that trientine may improve cardiac structural remodeling and function to a certain extent by improving oxidative stress and reducing myocardial damage caused by excess copper ions.

However, the research on trientine in the treatment of heart failure is still at an early stage, clinical evidence is limited and it has not yet been widely included in heart failure treatment guidelines. At present, this drug has not become a standard recommended drug for the treatment of heart failure, and is mainly used as a scientific research drug or exploratory treatment. For most patients with heart failure, the mainstream treatment is still ACEI/ARB, beta blockers, diuretics and other traditional drugs. Whether trientine is suitable for use requires comprehensive judgment under the guidance of a professional doctor.
Overall, although trientine hydrochloride shows certain potential in the treatment of heart failure, especially for types of heart failure under specific mechanisms, it cannot yet be promoted as a routine treatment. In the future, more randomized controlled clinical trials are needed to verify its safety and efficacy, and to identify the patient groups for which it is most suitable. For patients with heart failure, they should never use trientine by themselves. They should carefully evaluate the pros and cons under the guidance of a doctor before deciding whether to try this drug.
