Can I stop taking pirfenidone/asrenaline after taking it? Analysis of the effects of long-term use and risk of discontinuation
As an anti-fibrotic drug for the treatment of idiopathic pulmonary fibrosis (IPF), Pirfenidone usually needs to be taken for a long time, and it is not recommended to stop the drug at will. IPF is a progressive lung disease. Once pulmonary fibrosis occurs, it is usually irreversible. The main role of pirfenidone is to delay the decline of lung function rather than completely curing the disease. Therefore, sudden discontinuation of medication may lead to accelerated deterioration of the condition, even a sharp decline in lung function, and an increased risk of death.
Clinical studies have found that patients who take pirfenidone for a long time significantly slow down the decline in vital capacity and the disease progresses more slowly. Once the drug is discontinued, the disease may get out of control and pulmonary fibrosis may accelerate. Some patients may feel no significant changes in a short period of time after stopping the drug, but as time goes by, the rate of deterioration of lung function may increase, leading to worsening dyspnea. Therefore, IPF patients should strictly follow the doctor’s recommendations for long-term maintenance treatment and should not stop taking the medication on their own.

However, some patients may experience intolerance reactions during medication, such as gastrointestinal discomfort, photosensitivity reactions, abnormal liver function, etc. In this case, patients should communicate with their doctor to adjust the dose or adopt corresponding management strategies based on the severity of the adverse reaction, rather than discontinuing the drug directly. For example, for patients with gastrointestinal discomfort, medication can be taken after meals to reduce food irritation to the gastrointestinal tract. For photosensitivity reactions, patients need to avoid sun exposure and use sun protection measures, such as applying sunscreen or wearing a sun hat. In addition, patients with abnormal liver function need to regularly monitor liver enzyme levels. If the abnormality is severe, the doctor may recommend adjusting the dosage or changing the treatment plan.
If the medication needs to be stopped due to special reasons, the dose should be gradually reduced under the guidance of a doctor rather than stopped suddenly to avoid a sharp rebound of the condition. After stopping the drug, patients still need to be followed regularly to monitor changes in lung function so that treatment can be restarted if necessary.
