Cemiplimab is an anti-PD-1 monoclonal antibody used to treat certain types of skin cancer and non-small cell lung cancer. As an immune checkpoint inhibitor, it may cause immune-related adverse reactions and requires close monitoring and management.

Immune-related skin toxicities are common and include rash, pruritus, and dry skin, and some patients may develop severe exfoliative dermatitis or bullous dermatosis. Patients should pay attention to skin changes, keep the skin clean and moisturized, and avoid using irritating skin care products. If the rash worsens, antihistamines or topical glucocorticoids can be used under the guidance of a doctor. Severe cases may require short-term systemic glucocorticoid treatment.

Cimepilimab may cause immune-mediated pneumonitis, manifested by cough, shortness of breath, or chest pain, which may progress to acute respiratory failure in severe cases. Patients should pay attention to changes in respiratory symptoms and seek medical attention immediately if they experience persistent dry cough or difficulty breathing. When immune-related pneumonia is diagnosed, the drug usually needs to be discontinued and treated with corticosteroids or immunosuppressants.
This drug may also cause endocrine disorders such as thyroid dysfunction (hypothyroidism or hyperthyroidism), adrenal insufficiency, or type 1 diabetes. Patients should monitor hormone levels regularly and seek medical attention promptly if symptoms such as persistent fatigue, weight changes, dizziness, or unusual thirst occur. Mild endocrine abnormalities can be managed with hormone replacement therapy, while severe cases may require long-term endocrine regulation.
