There are no data on the presence of tofacitinib in breast milk, the effects on breastfed infants, or the effects on milk production, and patients are not advised to breastfeed during treatment and for at least 18 hours after the last dose of tofacitinib/tofacitinib oral solution or for 36 hours after the last dose of tofacitinib.

In animal reproduction studies, when pregnant rats and rabbits received tofacitinib during organogenesis at exposures 73 and 6.3 times the maximum recommended dose of 10 mg twice daily, fetocidal and teratogenic effects were observed, respectively. Additionally, in perinatal and postnatal studies in rats, tofacitinib resulted in decreased live births, postnatal survival, and pup body weight at exposures that were approximately 73 times the recommended dose of 5 mg twice daily and approximately 36 times the maximum recommended dose of 10 mg twice daily.
