Rivaroxaban dosage for the treatment of pulmonary embolism

Rivaroxaban is used to treat pulmonary embolism (PE). When CrCl ≥ 15 mL/min, take 15 mg twice a day, and then transition to 20 mg once a day after 21 days; when CrCl < 15 mL/min, avoid using rivaroxaban. Rivaroxaban is also indicated to reduce the risk of DVT and/or PE recurrence in adult patients who remain at risk for DVT and/or PE recurrence after completing initial treatment lasting at least 6 months, when CrCl ≥ 15 mL/min, 10 mg once daily after at least 6 months of standard anticoagulant therapy; avoid rivaroxaban when CrCl < 15 mL/min. Continue taking rivaroxaban even if you feel well. Do not stop taking rivaroxaban without talking to your doctor.
