Can Rivaroxaban be replaced with Warfarin?

Rivaroxaban and warfarin (Jantoven) are oral blood thinners used to treat and prevent blood clots. Unlike warfarin, rivaroxaban does not require frequent blood tests and strict dietary considerations, and it has been shown to be as effective as warfarin. Rivaroxaban is available as tablets and granules for preparation of oral suspension, and the dose and duration of treatment depend on its use and the patient's risk of bleeding. In children, the form, dose, and duration of treatment also depend on the patient's age and weight. When used in combination with antiplatelet drugs such as aspirin, clopidogrel, or ticlopidine, the dose of rivaroxaban is lower (2.5 mg twice daily).
