Vigabatrin is mainly used to treat partial seizures of epilepsy. It can also be used in combination with other anti-epileptic drugs to treat refractory epileptic seizures. It can also be used to treat Lennox-Gastaut (a refractory epilepsy) and West (infant spasms) syndrome in children. Vigabatrin may cause GABA-mediated retinal axonal cytotoxicity, causing irreversible visual field defects in approximately 30% to 60% of patients. However, it plays an irreplaceable role in the treatment of intractable epilepsy.
Vigabatrin dosage:
1. Routine dosage for adults:
Oral administration, anti-epileptic: The recommended dose is 3g per day, taken in divided doses. The dose should be increased according to the clinical effect at a rate of 0.5-1g per day to 4g per day (or 50mg/kg per day), which is effective for complex partial seizures of epilepsy (with or without generalized tonic-clonic seizures).
2. Dosage for renal insufficiency:
For patients whose creatinine clearance is less than or equal to 60ml/min, it is recommended to reduce the dose.
3. Routine dosage for children:
1. General usage: For children aged 3-9 years, the recommended dosage is 1g per day, taken in 2 times; for older children, the recommended dosage is 2g per day, divided into 2 times.
2. West syndrome: The average oral dose is 100mg/kg per day (range: 50-150mg/kg).
Adverse reactions of Vigabatrin:
(1) Drowsiness, ataxia, headache, dizziness, agitation, memory impairment and weight gain may occur, but most patients do not see these adverse reactions.
(2) When combined with other drugs, Vigabatrin can reduce the plasma concentrations of phenytoin, phenobarbital and primidone by 20%, 7% and 11% respectively. When this drug is combined with phenytoin, the dose of phenytoin needs to be increased (but there is no need to increase the dose when phenobarbital and primidone are combined with this product).
